Required Match Day Paperwork
2026 Match Day Paperwork Requirements – The following documents must be submitted to the league post-match.
UMPIRES: The following must be submitted no later than 7pm post-match.
- Home Club Team Sheet – (from PlayHQ ONLY, no handwritten Team Sheets will be accepted)
- Away Club Team Sheet – (from PlayHQ ONLY, no handwritten Team Sheets will be accepted)
- Timekeepers Report – Click here to download

CLUBS:
- Print Team Sheet from PlayHQ. Handwritten team sheets will not be accepted. Changes on the day may be handwritten on printed PlayHQ team sheet. Team Sheets must be signed by the players, if not signed they will not be credited with that match.
- Must update PlayHQ scores, team sheets and goal kickers by 12pm Monday, post-match.
- Marsh Insurance Matchday Checklist. This must be completed prior to the days play – Click here to complete form
